[1] 00:15There's actually a major health crisis today

[2] 00:18in terms of the shortage of organs.
em termos da escassez de órgãos.

[3] 00:20The fact is that we're living longer.

[4] 00:22Medicine has done a much better job

[5] 00:24of making us live longer,
nos fazedo viver por mais tempo.

[6] 00:26and the problem is, as we age,
E o problema é que, à medida que envelhecemos,

[7] 00:28our organs tend to fail more,
nossos órgãos tendem a falhar mais.

[8] 00:31and so currently
E assim hoje

[9] 00:33there are not enough organs to go around.
não há órgãos suficientes para todos.

[10] 00:35In fact, in the last 10 years,
De fato, nos últimos 10 anos,

[11] 00:37the number of patients requiring an organ has doubled,

[12] 00:40while in the same time,
enquanto que no mesmo tempo,

[13] 00:42the actual number of transplants has barely gone up.
o número de transplantes mal subiu.

[14] 00:45So this is now a public health crisis.

[15] 00:47So that's where this field comes in
Então é aí que este campo

[16] 00:49that we call the field of regenerative medicine.
que chamamos de medicina regenerativa entra.

[17] 00:51It really involves many different areas.
Ele envolve diferentes áreas.

[18] 00:53You can use, actually, scaffolds,
Você pode usar moldes,

[19] 00:55biomaterials --
biomateriais -

[20] 00:57they're like the piece of your blouse or your shirt --

[21] 00:59but specific materials you can actually implant in patients

[22] 01:02and they will do well and help you regenerate.
e eles vão funcionar e ajudarão na regeneração.

[23] 01:05Or we can use cells alone,
Ou podemos usar apenas células

[24] 01:08either your very own cells
tanto as suas próprias células

[25] 01:10or different stem cell populations.
como as de células-tronco.

[26] 01:12Or we can use both.
Ou podemos usar os dois;

[27] 01:14We can use, actually, biomaterials
podemos usar biomateriais

[28] 01:16and the cells together.
e as células juntos.

[29] 01:18And that's where the field is today.
E é aí que o campo está hoje.

[30] 01:21But it's actually not a new field.

[31] 01:23Interestingly, this is a book
Curiosamente, este é um livro

[32] 01:25that was published back in 1938.
que foi publicado em 1938.

[33] 01:28It's titled "The Culture of Organs."
Seu título é "A Cultura de Órgãos".

[34] 01:30The first author, Alexis Carrel, a Nobel Prize winner.
O primeiro autor, Alexis Carrel, recebeu o prêmio Nobel.

[35] 01:33He actually devised some of the same technologies

[36] 01:35used today for suturing blood vessels,
usadas hoje para suturar vasos sanguíneos.

[37] 01:37and some of the blood vessel grafts we use today

[38] 01:40were actually designed by Alexis.
foram desenhado por Alexis.

[39] 01:43But I want you to note his co-author:
Mas eu quero que você observe seu co-autor:

[40] 01:46Charles Lindbergh.
Charles Lindbergh.

[41] 01:48That's the same Charles Lindbergh
É o mesmo Charles Lindbergh

[42] 01:51who actually spent the rest of his life
que passou o resto de sua vida

[43] 01:53working with Alexis
trabalhando com Alexis

[44] 01:55at the Rockefeller Institute in New York
no Instituto Rockefeller, em Nova York

[45] 01:57in the area of the culture of organs.
na área da cultura de órgãos.

[46] 01:59So if the field's been around for so long,

[47] 02:01why so few clinical advances?
por que ocorreram tão poucos avanços clínicos?

[48] 02:03And that really has to do to many different challenges.

[49] 02:06But if I were to point to three challenges,
Mas se eu fosse apontar três desafios,

[50] 02:08the first one is actually the design of materials
o primeiro é o design de materiais

[51] 02:10that could go in your body
que poderiam ser implantados em seu corpo

[52] 02:12and do well over time.
e funcionar corretamente ao longo do tempo.

[53] 02:14And many advances now,
E com muitos avanços,

[54] 02:16we can do that fairly readily.
agora podemos fazer isso muito facilmente.

[55] 02:18The second challenge was cells.
O segundo desafio foram as células.

[56] 02:20We could not get enough of your cells to grow outside of your body.

[57] 02:23Over the last 20 years, we've basically tackled that.
Nos últimos 20 anos, nós basicamente resolvemos isso.

[58] 02:26Many scientists can now grow many different types of cells.

[59] 02:28Plus we have stem cells.
e ainda temos as células-tronco.

[60] 02:30But even now, 2011,
Mas mesmo agora, em 2011,

[61] 02:33there's still certain cells that we just can't grow from the patient.

[62] 02:37Liver cells, nerve cells, pancreatic cells --
As células do fígado, as células nervosas, as células pancreáticas -

[63] 02:40we still can't grow them even today.
nós ainda não podemos cultivá-las.

[64] 02:43And the third challenge is vascularity,
E o terceiro desafio é a vascularização

[65] 02:45the actual supply of blood
e o fornecimento de sangue real

[66] 02:48to allow those organs or tissues to survive

[67] 02:51once we regenerate them.
depois de regenerados.

[68] 02:53So we can actually use biomaterials now.
Portanto, nós podemos utilizar biomateriais agora.

[69] 02:55This is actually a biomaterial.
Este é um biomaterial.

[70] 02:57We can weave them, knit them, or we can make them like you see here.
Nós podemos tecê-los, tricotá-los, ou nós podemos desenvolvê-los como você vê aqui.

[71] 03:00This is actually like a cotton candy machine.

[72] 03:03You saw the spray going in.
Você viu o spray entrando.

[73] 03:05That was like the fibers of the cotton candy

[74] 03:07creating this structure, this tubularized structure,
criando essa estrutura, essa estrutura tubular.

[75] 03:09which is a biomaterial
que é um biomaterial

[76] 03:11that we can then use
que podemos usar

[77] 03:13to help your body regenerate
para ajudar seu corpo a regenerar

[78] 03:15using your very own cells to do so.
usando suas próprias células para isso.

[79] 03:18And that's exactly what we did here.

[80] 03:20This is actually a patient
Este é um paciente

[81] 03:22who [was] presented with a deceased organ,
que apresentou um órgão falecido,

[82] 03:24and we then created one of these smart biomaterials,
e então nós criamos um desses biomateriais inteligentes,

[83] 03:27and then we then used that smart biomaterial
e, em seguida nós usamos esse material

[84] 03:29to replace and repair
para substituir e raparar

[85] 03:32that patient's structure.
o órgão desse paciente.

[86] 03:34What we did was we actually
O que nós fizemos foi na verdade

[87] 03:36used the biomaterial as a bridge
o uso do biomaterial como uma ponte

[88] 03:38so that the cells in the organ
de modo que as células do órgão

[89] 03:40could walk on that bridge, if you will,
poderiam andar sobre a ponte,

[90] 03:42and help to bridge the gap
e ajudar a preencher o vazio

[91] 03:44to regenerate that tissue.
para regenerar esse tecido.

[92] 03:46And you see that patient now six months after
E você vê aquele paciente agora, seis meses depois,

[93] 03:48with an X-ray showing you the regenerated tissue,

[94] 03:51which is fully regenerated
que está totalmente regenerado

[95] 03:53when you analyze it under the microscope.
quando você o analisa pelo microscópio.

[96] 03:56We can also use cells alone.
Podemos também usar apenas células.

[97] 03:58These are actually cells that we obtained.
Estas são células que nós obtivemos.

[98] 04:01These are stem cells that we create from specific sources,

[99] 04:04and we can drive them to become heart cells,

[100] 04:06and they start beating in culture.
E eles começam a bater dentro da cultura.

[101] 04:08So they know what to do.
Então elas sabem o que fazer.

[102] 04:10The cells genetically know what to do,
As células geneticamente sabem o que fazer,

[103] 04:12and they start beating together.
e eles começam a bater juntas.

[104] 04:14Now today, many clinical trials
E hoje, muitos ensaios clínicos

[105] 04:16are using different kinds of stem cells
estão usando diferentes tipos de células-tronco

[106] 04:18for heart disease.
para doenças cardíacas.

[107] 04:20So that's actually now in patients.
Então isso está nos pacientes agora.

[108] 04:23Or if we're going to use larger structures
Ou, se utilizarmos estruturas maiores

[109] 04:25to replace larger structures,
para substituir as estruturas maiores,

[110] 04:27we can then use the patient's own cells,
podemos então usar as células do próprio paciente,

[111] 04:29or some cell population,
ou alguma população de células,

[112] 04:31and the biomaterials,
e os biomateriais,

[113] 04:33the scaffolds, together.
os moldes, juntos.

[114] 04:35So the concept here:
Então aqui está o conceito:

[115] 04:37so if you do have a deceased or injured organ,

[116] 04:40we take a very small piece of that tissue,
nós retiramos um pequeno pedaço desse tecido,

[117] 04:42less than half the size of a postage stamp.

[118] 04:45We then tease the cells apart,
Nós então isolamos as células

[119] 04:48we grow the cells outside the body.
e fazemos elas se desenvolverem fora do corpo.

[120] 04:50We then take a scaffold, a biomaterial --
Nós pegamos a matriz, o biomaterial,

[121] 04:53again, looks very much like a piece of your blouse or your shirt --

[122] 04:56we then shape that material,
Depois nós damos forma ao material,

[123] 04:58and we then use those cells to coat that material

[124] 05:01one layer at a time --
uma camada de cada vez -

[125] 05:03very much like baking a layer cake, if you will.
é como fazer um bolo de camadas.

[126] 05:06We then place it in an oven-like device,
Nós o colocamos num dispositivo para aquecê-lo

[127] 05:08and we're able to create that structure
e assim nós conseguimos criar essa estrutura

[128] 05:10and bring it out.
e trazê-la para fora.

[129] 05:12This is actually a heart valve
Esta é uma válvula cardíaca

[130] 05:14that we've engineered,
que nós construímos.

[131] 05:16and you can see here, we have the structure of the heart valve

[132] 05:19and we've seeded that with cells,
nós a semeamos com células

[133] 05:22and then we exercise it.
e então exercitamos ela.

[134] 05:24So you see the leaflets opening and closing --
Você vê os folhetos abrindo e fechando -

[135] 05:26of this heart valve
dessa válvula cardíaca

[136] 05:28that's currently being used experimentally
que atualmente está sendo usada experimentalmente

[137] 05:32to try to get it to further studies.
para tentar levá-la para estudos posteriores.

[138] 05:35Another technology
Outra tecnologia

[139] 05:37that we have used in patients
que temos utilizado em pacientes

[140] 05:39actually involves bladders.
envolve bexigas.

[141] 05:41We actually take a very small piece of the bladder from the patient --

[142] 05:44less than half the size of a postage stamp.

[143] 05:47We then grow the cells outside the body,
E então nós desenvolvemos as células fora do corpo

[144] 05:49take the scaffold, coat the scaffold with the cells --
pegamos o molde, vestimos com células -

[145] 05:51the patient's own cells, two different cell types.

[146] 05:54We then put it in this oven-like device.

[147] 05:57It has the same conditions as the human body --

[148] 05:5937 degrees centigrade, 95 percent oxygen.

[149] 06:02A few weeks later, you have your engineered organ

[150] 06:05that we're able to implant back into the patient.

[151] 06:08For these specific patients, we actually just suture these materials.
Para esses pacientes específicos, nós apenas suturamos esses materiais.

[152] 06:11We use three-dimensional imagining analysis,
Nós fazemos análises a partir de imagens tridimensionais

[153] 06:14but we actually created these biomaterials by hand.

[154] 06:18But we now have better ways
Mas agora temos métodos melhores

[155] 06:20to create these structures with the cells.
para criar essas estruturas com as células.

[156] 06:22We use now some type of technologies,
Agora nós usamos algumas tecnologiais

[157] 06:26where for solid organs, for example,
que para órgãos sólidos, como por exemplo

[158] 06:28like the liver,
o figado,

[159] 06:30what we do is we take discard livers.
o que fazemos é usar fígados descartados.

[160] 06:33As you know, a lot of organs are actually discarded, not used.

[161] 06:36So we can take these liver structures,
Então nós podemos pegar esses fígados

[162] 06:38which are not going to be used,
que não serão utilizados,

[163] 06:40and we then put them in a washing machine-like structure

[164] 06:44that will allow the cells to be washed away.

[165] 06:46Two weeks later,
Duas semanas depois,

[166] 06:48you have something that looks like a liver.

[167] 06:50You can hold it like a liver,
Você pode segurá-lo como um fígado,

[168] 06:52but it has no cells; it's just a skeleton of the liver.

[169] 06:55And we then can re-perfuse the liver with cells,
E então nós podemos irrigar o figado com células

[170] 06:59preserving the blood vessel tree.
preservando os vasos sanguíneos.

[171] 07:01So we actually perfuse first the blood vessel tree
Então nós perfundimos primeiro a árvore vascular

[172] 07:04with the patient's own blood vessel cells,
com células vasculares do próprio paciente,

[173] 07:06and we then infiltrate the parenchyma with the liver cells.
e então nós infiltramos o parênquima com células hepáticas.

[174] 07:09And we now have been able just to show
E então nós somos capazes de mostrar

[175] 07:11the creation of human liver tissue
a criação do tecido hepático humano

[176] 07:13just this past month
exatamente no mês passado

[177] 07:15using this technology.
usando essa tecnologia.

[178] 07:19Another technology that we've used
Outra tecnologia que nós usamos

[179] 07:21is actually that of printing.
é a da impressão.

[180] 07:23This is actually a desktop inkjet printer,

[181] 07:26but instead of using ink,
mas ao invés de usar tinta,

[182] 07:28we're using cells.
estamos usando células.

[183] 07:30And you can actually see here the printhead

[184] 07:32going through and printing this structure,
passando e imprimindo essa estrutura,

[185] 07:34and it takes about 40 minutes to print this structure.
e demora cerca de 40 minutos para imprimi-la.

[186] 07:36And there's a 3D elevator
E há um elevador 3D

[187] 07:38that then actually goes down one layer at a time

[188] 07:40each time the printhead goes through.

[189] 07:42And then finally you're able to get that structure out.

[190] 07:45You can pop that structure out of the printer and implant it.

[191] 07:48And this is actually a piece of bone
E este é um pedaço de osso

[192] 07:50that I'm going to show you in this slide
que eu vou mostrar neste slide

[193] 07:53that was actually created with this desktop printer
que foi criado com uma impressora

[194] 07:56and implanted as you see here.
e implantado como você pode ver aqui.

[195] 07:59That was all new bone that was implanted

[196] 08:01using these techniques.
utilizando essas técnicas.

[197] 08:04Another more advanced technology we're looking at right now,

[198] 08:08our next generation of technologies,
nossa próxima geração de tecnologias,

[199] 08:10are more sophisticated printers.
são as impressoras mais sofisticadas.

[200] 08:12This particular printer we're designing now

[201] 08:15is actually one where we print right on the patient.

[202] 08:18So what you see here --
Então o que você vê aqui -

[203] 08:20I know it sounds funny,
Eu sei que soa engraçado,

[204] 08:22but that's the way it works.

[205] 08:24Because in reality, what you want to do

[206] 08:27is you actually want to have the patient on the bed with the wound,

[207] 08:30and you have a scanner,
e você com um scanner,

[208] 08:32basically like a flatbed scanner.
basicamente como um scanner de mesa.

[209] 08:34That's what you see here on the right side.
Isso é o que você vê aqui no lado direito;

[210] 08:36You see a scanner technology
você vê uma tecnologia de scanner

[211] 08:38that first scans the wound on the patient
que primeiro examina a ferida do paciente

[212] 08:41and then it comes back with the printheads

[213] 08:44actually printing the layers that you require
imprimindo as camadas que você necessitar

[214] 08:47on the patients themselves.
sobre os próprios pacientes.

[215] 08:49This is how it actually works.
É assim que ele funciona.

[216] 08:51Here's the scanner going through,
Aqui está o scanner passando

[217] 08:53scanning the wound.
e digitalizando a ferida.

[218] 08:55Once it's scanned,
Uma vez identificada,

[219] 08:57it sends information in the correct layers of cells
ele envia informações sobre as camadas de células

[220] 08:59where they need to be.
exatamente onde eles precisam estar

[221] 09:01And now you're going to see here
E agora você vai ver aqui

[222] 09:03a demo of this actually being done
uma demonstração disso acontecendo

[223] 09:05in a representative wound.
em uma representação de uma ferida.

[224] 09:08And we actually do this with a gel so that you can lift the gel material.

[225] 09:11So once those cells are on the patient

[226] 09:13they will stick where they need to be.
eles vão ficar onde eles precisam estar.

[227] 09:15And this is actually new technology
E essa é uma nova tecnologia

[228] 09:17still under development.
ainda em desenvolvimento.

[229] 09:20We're also working on more sophisticated printers.
Também estamos trabalhando em impressoras mais sofisticadas.

[230] 09:23Because in reality, our biggest challenge
Porque na verdade nosso maior desafio

[231] 09:25are the solid organs.
são os órgãos sólidos.

[232] 09:27I don't know if you realize this,
Eu não sei se vocês sabem disso,

[233] 09:29but 90 percent of the patients on the transplant list

[234] 09:33are actually waiting for a kidney.
estão esperando por um rim.

[235] 09:36Patients are dying every day
Pacientes morrem todos os dias

[236] 09:38because we don't have enough of those organs to go around.

[237] 09:41So this is more challenging --
Então isso é mais difícil -

[238] 09:43large organ, vascular,
um órgão grande, vascular,

[239] 09:45a lot of blood vessel supply,
um grande suprimento de vasos sanguíneos

[240] 09:47a lot of cells present.
muitas células presentes.

[241] 09:49So the strategy here is --
Assim, a estratégia aqui é -

[242] 09:51this is actually a CT scan, an X-ray --
essa é uma tomografia computadorizada, um raio-X -

[243] 09:53and we go layer by layer,
e nós vamos camada por camada,

[244] 09:55using computerized morphometric imaging analysis

[245] 09:57and 3D reconstruction
e reconstrução 3D

[246] 09:59to get right down to those patient's own kidneys.
para ir direto aos rins do paciente.

[247] 10:02We then are able to actually image those,
E então nós somos capazes de digitalizá-los,

[248] 10:05do 360 degree rotation
fazer uma rotação de 360 graus

[249] 10:07to analyze the kidney
para analisar o rim

[250] 10:09in its full volumetric characteristics,
em todas as suas características volumétricas,

[251] 10:13and we then are able
e então somos capazes

[252] 10:15to actually take this information
de obter essa informação

[253] 10:17and then scan this
e digitalizá-la

[254] 10:19in a printing computerized form.
de forma que possa ser impressa.

[255] 10:21So we go layer by layer through the organ,
Nós vamos camada por camada
